Evaluating Care Needs
Evaluate, arrange and monitor at home or medical care needs of clients. Explain complex and difficult topics with the clients or family members.
Coordinating Medical/Social Needs
Arranging for transportation to medical appointments , identifying social services, creating short and long term care plans , that could help the care of your our clients.
Acting as liaison
Act as liasion for families who live too far away or others who often have other commitments such as parenting or workplace responsibilities. Answer questions and concerns about our clients.

Assessment and Plan of Care
Evaluation of current needs and develop an individual Care Plan for your loved one. This will serve as a road map for resources and services required
Continuous Care
Evaluation of current needs and develop an individual Care Plan for your loved one. This will serve as a road map for resources and services required
Medication Management
Our Care Manager is able to set-up and monitor medication for your loved one. This includes managing refills, and communicating with doctors as needed.
Relocation Services
I will assist you in relocating your loved one; whether it be moving to another city, or moving your loved one closer to you. We are also able to assist in moving your loved one to a more appropriate residence which better meets their needs.
Crisis Intervention
Crisis Intervention includes an Assessment and Care Plan, but is expedited in a much more rapid time frame due to the nature of the situation. Our Nurse Care Manager is available 24/7 for crisis situations. Behavioral outburst, Wandering, Hospital or rehabilitation discharge, Mental health issues, Family conflict, Exploitation, Abuse or neglect, Safety issues
